Arizona TASH Legacy Award

 

Award Description:

Each year, Arizona TASH recognizes one individual, program or organization for their efforts to promote inclusive practices in Arizona schools, especially for school-aged students who have moderate to severe disabilities. Each year, Arizona TASH will make only one award.

 

The AzTASH Legacy Award comes with a special “Pulling for All Kids” award, a one-year membership to TASH, plus a $1000 cash award. The award recipient will be announced at the 2010 Annual Arizona TASH Conference on Inclusive Practices.

 

Eligibility Requirements:

·        No self-nominations will be accepted.

·        Nominee’s work must have positively impacted inclusive practices for students in Arizona who have moderate to severe disabilities.

·        Nominee’s work must have involved a number of classrooms, students, teachers, parents, and/or community members.

·        Nominee’s work must demonstrate a sustained effort that is likely to continue.

·        Nominee must be available for phone interview and/or a visit by AzTASH Legacy Award committee members.

 

How to nominate someone, a program, or an organization:

·        Complete the attached nomination form and submit via email to Andrea O’Brien (andrea.m.obrien@cox.net) by no later than April 15, 2010.

·        Send/fax any supporting documentation (news articles, description of project/program/organization, etc.), if available to:
